Immunization Policy/Requirements

Immunizations are the most effective means of preventing many serious communicable diseases and are required for school attendance.

Proof of immunizations must be provided on an official Kansas Certificate of Immunization KCI Form, or an acceptable Computerized Immunization Record, or an acceptable stamped “Pink Book” and presented to your school at the time of initial enrollment.

USD 428 Board Policy JGCB Immunizations and inoculations, states,” Students who do not have a complete set of immunizations will have fourteen (14) days from the time of notification to provide verification, or will be excluded from school until which time verifications is provided.”  

State law requires a complete series of immunizations for admission to school.
